In-Home Assessments

The purpose of an in-home assessment is to help families determine how to create a safer living environment for the individual living with Alzheimer's or Dementia. Often families have a difficult time understanding the disease and what the needs of their loved one might be now and in the future.

Each Assessment includes:

  • A Needs Assessment/Evaluation
  • A Home Safety Evaluation
  • Recommendations for Care
  • A Written Report with Documentation of Findings
